Full-Time LPN Case Manager - Springfield
Location: In-home assessments in Greater Springfield
Schedule: Monday–Friday | 9:00 AM–5:00 PM
Assignment: 13 weeks to start
Temp-to-Perm & Direct-Placement options available
Compensation
Pay Rate: $47/hour
Mileage Reimbursement: $0.72 per mile
Consistent weekday schedule — no nights, weekends, or holidays
Position Overview
Amergis Healthcare Staffing is partnering with a well-respected Adult Day Health organization that is committed to helping older adults and individuals with disabilities remain safe, independent, and supported in their homes. Through individualized care planning, clinical oversight, caregiver education, and comprehensive case management, this organization delivers high-quality, community-focused care throughout the Greater Springfield area.
As an LP N Case Manager , you will take a lead role in conducting clinical assessments, monitoring ongoing needs, and coordinating services to ensure continuity of care. You’ll collaborate closely with an interdisciplinary Care Team, caregivers, and healthcare providers to develop proactive, patient-centered care plans that promote safety, dignity, and quality of life.
This role is ideal for an LPN who values autonomy, relationship-based care, and meaningful community impact —without the demands of inpatient or shift-based work.
Key Responsibilities
Conduct intake, admission, and annual clinical assessments in client homes.
Complete accurate, timely documentation including assessments, care plans, and coordination with PCPs and other healthcare providers.
Educate caregivers on admission materials, care communication tools, and EVV systems.
Perform regular in-home visits to assess health status, environmental safety, and caregiver competency.
Conduct semi-annual visits including medication review and reconciliation, care plan updates, and caregiver communication tool audits.
Identify and address concerns related to safety, psychosocial needs, or changes in medical condition.
Participate in interdisciplinary team meetings to ensure coordinated, high-quality care.
Provide compassionate, culturally sensitive care while maintaining a strong customer-service mindset.
Perform additional duties within RN scope of practice as assigned.
Qualifications
Active Massachusetts LPN license in good standing (required).
2+ years of nursing experience preferred (new grads encouraged to apply).
Strong clinical assessment skills (required).
Ability to work independently and manage time effectively in community-based settings.
Sound clinical judgment and strong critical-thinking skills.
Intermediate to advanced proficiency with Microsoft Office Suite .
Experience with EMRs and clinical documentation systems .
Ability to use telehealth tools (video, chat, etc.) when appropriate.
Previous case management experience preferred but not required.
Valid driver’s license with reliable transportation and ability to travel throughout assigned service areas.
Working Conditions
Work performed in residential homes and community settings.
Member-facing role with regular in-person client interactions.
Occasional remote documentation time available.
Local travel required during the workday.
