Home Health Physical Therapist | PRN Opportunity

Your Role as a Physical Therapist
Deliver high-quality, patient-centered care aligned with Elara Caring’s mission
Assess patient needs, develop care plans, and monitor progress
Collaborate with interdisciplinary teams to optimize outcomes
Adjust treatment plans based on patient progress and goals
Supervise and guide support staff
Maintain accurate and timely clinical documentation
Promote physical therapy services within the community
Qualifications
Graduate of an APTA-accredited Physical Therapy program
Current, unrestricted PT license in the state of practice
At least 1 year of experience in a healthcare setting
Willingness and ability to travel within the service area (up to 50%)
Valid driver’s license, reliable transportation, and current auto insurance
Ability to lift, push, pull, or support up to 50 pounds
You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
