? What You’ll Do
As a Home Health RN, you’ll be the heart of patient care—bringing clinical excellence directly into patients’ homes.
Manage a dynamic caseload of patients in your local community
Perform comprehensive assessments and develop individualized care plans
Provide hands-on skilled nursing care and patient education
Monitor patient progress and adjust care plans as needed
Collaborate with physicians, therapists, and interdisciplinary teams
Be a trusted partner for patients and families throughout their care journey
? What You’ll Need
Active, unrestricted RN license (Connecticut)
At least 1 year of clinical nursing experience
Ability to travel locally (you’ll spend time in the field—this is not a desk job)
Valid driver’s license, reliable vehicle, and current auto insurance
Ability to safely assist patients (lifting, positioning, etc.)
Preferred (but not required):
Home Health or Hospice experience
Familiarity with HomeCare HomeBase
Associate’s or Bachelor’s Degree in Nursing
