Platte County Legacy Home Charge Nurse (RNs and LPNs) in Wheatland, Wyoming

JOB SUMMARY:

*The Charge Nurse assesses, plans, implements, evaluates and documents nursing care of residents in accordance with departmental and facility policies and with standards of professional nursing practice. This position is accountable for the quality of nursing services delivered by self and others who are under his/her direction.        *

ESSENTIAL FUNCTIONS:

Provides comprehensive coordinated, safe care of the resident and family using a multidisciplinary approach.  Is accountable and responsible for implementing established standards of care and evidence based practice to ensure the quality of nursing care provided at PCMNH.

  • Follows and documents a plan of care, including the discharge plan, utilizing assessment data, resident, and family with care team input.
  • Implements  the plan of care through direct and indirect resident care
  • *Evaluates the resident's and family's response to care.   *
  • Documents assessment, planning, implementation and evaluation of care. Including monthly summary of resident status as assigned. Documentation is timely, accurate and complete
  • Follows standard precautions in the provision of patient care and implements other infection control measures as necessary.
  • Utilizes equipment according to manufacturer's recommendations and hospital policy.
  • Passes medications promptly and safely following facility protocol, monitors appropriately for adverse drug reactions
  • *Provides a climate in which individuals and families may act in their own interest. The Charge nurse intervenes when residents are unable to act in their own interest.      *
  • Communicates with other health care providers the resident and family wishes/requests as the plan of care requires changes.
  • Document the resident's /family's wishes to effectively communicate with all other health care providers.
  • Reports/removes unsafe equipment following PCMNH facility guidelines.
  • Delegates appropriately, and provides nursing supervision in the provision of care to residents by certified nursing assistants and other personnel.
  • *Manages the team assigned during shift to assure resident cares are complete. *
  • * Assures dining room flow, cleaning schedules, toileting schedules, turning and repositioning schedules are followed according to policy or individual care plan.*
  • Promotes continuity of care by accurately and completely communicating to other caregivers the status of residents for whom care is provided.
  • Reports/documents unexpected occurrences involving residents and others as appropriate.
  • *Follows PCMNH emergency and disaster preparedness plan.  *
  • Ensures care is provided on basis of physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements
  • Other duties as assigned

PHYSICAL/MENTAL DEMANDS/ ENVIRONMENT:

  • Exposed regularly to infection within resident rooms
  • Must be on constant alert to the needs of residents; including possible falls or dangers that may cause injury
  • Exposed to odors
  • Exposed to physical & emotional disabilities of the aged, which may include combative residents
  • Standing, walking, squatting or kneeling,  transferring and pushing wheelchair bound residents and lifting up to 25 lbs
  • 3-5 hours of continual computer work
  • *Must be able to speak, read, write in English; calculate basic math and algebraic equations; see, hear, use fine motor skills, eye-hand coordination, and tactile abilities. *
  • *May be required to work in stressful situations requiring quick thinking and coordination of a variety of activities at once.  *
  • There is potential exposure to hazardous substances
  • Is prompt and efficient with minimal absences.

EQUIPMENT USED:

Electronic and manual blood pressure equipment, pulse oximetery, glucometer, electric bed, stretcher, portable suction, whirlpool tub, wheelchairs, commodes,

stand-up and full body lifts, hepa filters, geri-chair, pneumatic mattress, infusion pumps, blanket warmer, nurse call system, bed alarms, aqua-K pumps, humidifier,

O2 tank and regulators, wander guard, trapeze, plexi pulse, gastric feeding pump, microwave, video and television, computers, printers, telephones, paper shredder and copiers

.

ADDITIONAL FUNCTIONS:

The Charge nurse has additional professional functions and responsibilities, which include understanding and facilitating the staffing process, maintains awareness of current unit, facility and system information updates; supports and participates in shared leadership process; completes mandatory educational requirements and recognizes own learning needs and seeks opportunities for improvement; directs concerns of clinical practice/operations through appropriate channels; and interacts and participates in the education, orientation of hospital personnel, patients, students, families and visitors. May function as a preceptor/mentor.

All other duties as assigned within the scope of practice of nursing.

MINIMUM QUALIFICATIONS:

*Must possess a current valid Wyoming LPN license with a current BLS.     *

*Experience in the clinical area for which he or she is applying is desired.    *
