Job Description:
The RN Comprehensive Care Coordinator is a Registered Nurse (RN) directly responsible for assessing, coordinating, and integrating healthcare for patients with complex or chronic conditions across the continuum of care. This RN position manages all aspects of patient centric care coordination through collaboration with physicians, staff, and other health professionals to assist patients to reach optimal function and health in an appropriate environment that supports their needs. The RN Comprehensive Care Coordinator will work directly with the entire health care team to deliver innovative population health management practices for patients with ongoing medical and/or social concerns leading to higher utilization of health care services.
Experience/Qualifications:
Knowledge of utilization review standards and patient care practices normally acquired through graduation of an accredited school of professional nursing.
A minimum of five years of acute care hospital experience.
Experience in critical care, general medical and/or surgical nursing, and experience in educational presentations beneficial, as well as previous experience in the application of Utilization Review activities and medical record reviews.
Possesses an active Wisconsin RN nursing license. Will need to pursue a Michigan nursing license.
A bachelor’s degree in nursing, successful completion of quality or Utilization Review certification examination, and active membership in professional organizations (i.e., NAHQ, WAHO) beneficial. Certification in Case Management desired.

Medicare system
MedStar initiatives
Utilization review principles and objectives
Managed care indicators
NCQA standards and requirements
Quality Improvement principles and Risk Management objectives
Possesses strong clinical skills, assessment, and good professional judgment, as well as broad-based knowledge of medical management, nursing management and medical terminology.
Demonstrates the ability to read and comprehend government regulations and contracts.
Demonstrates familiarity and knowledge of medical records organization and procedures.
Skills in organizing and prioritizing work along with multi-tasking simultaneously.
Skills in working effectively as a dedicated, creative, and resourceful team player.
Possesses excellent oral and written communication skills as well as excellent interpersonal and public relations skills.
Demonstrates sound clinical judgment, decision-making, and problem-solving skills.
Willingness to be a change agent and positive role model.
Knowledge of motivational communication techniques to engage patients/families.
Ability to demonstrate a strong, autonomous work ethic with qualities of personal and professional integrity.
Detail and goal oriented demonstrated by personal initiative and accountability.
Possesses confidence to communicate and outreach to other community health care organizations and personnel.
Possesses proficient computer skills, including Microsoft Office (specifically Word and Excel).
