SUMMARY OF JOB RESPONSIBILITIES
Personal Support Aide services refer to those tasks designed to assist individuals with home-related activities such as light housekeeping, basic personal care needs and basic home maintenance.
These services are directed by the Nursing Supervisor based on the care plan.
QUALIFICATIONS
1.
High school graduates are preferred, but educational requirements may be waived if applicant can follow written and verbal instructions, and complete reports.
2.
Must have the following training and/or experience by:
A.
Successful completion of a nurse aide training and competency evaluation program OR
B.
Successful completion of a competency examination for nurse Aides recognized by the Department of Community Health, or
C.
Successful completion of a health care or personal care credentialing program recognized and approved by the Department of Community Health, or
D.
Successful completion or progress in the completion of a 40-hour training program provided by a private home care provider.
3.
Minimum age of 18 years.
4.
Must be flexible and tolerant of varied lifestyles.
5.
Must be able to work under supervision and within the guidelines of a care plan.
6.
Must be in good physical and mental health.
7.
Demonstrates tact, patience, and good personal hygiene.
8.
Is a licensed driver with an automobile that is insured in accordance with agency requirements and is in good working order.
RESPONSIBILITIES AND DUTIES
1.
Assistance with bathing, shampooing, toiletry, grooming, shaving, oral hygiene, feeding and dressing.
2.
Assistance with ambulation of clients, transfer of client, and positioning of client.
3.
Remind/assist client with self-administration of medications.
4.
Meal preparation and serving.
5.
Apply basic first aid/CPR in case of sudden illness or accident.
6.
Encourage family members to be involved with care of the client.
7.
Maintain accurate and legible records of visits and services rendered, indicating changes in the clients condition, additional needs of client or any problems that hinder service delivery.
8.
Provide services according to the service care plan directing any questions to the Nursing Supervisor or Administrator.
9.
Check in and out with the Aide Scheduler /designee each day as required by company policy.
10.
Performs housekeeping duties to include sanitation and home safety.
11.
Completes laundry as appropriate.
12.
Adheres to company standards of personal and professional conduct.
13.
Reports any accident, incident, concerns, or changes in the clients condition to the Administrator, Nursing Supervisor and Aide scheduler immediately.
