Wyoming County Human Resources Case Manager - Office for the Aging in Warsaw, New York
CASE MANAGER: OFFICE FOR THE AGING
HOURS: 35 hours per week - Full Time
SALARY:$24.65 $28.42 per hour
LOCATION:Wyoming County Office for the Aging 8 Perry Avenue, Warsaw, NY 14569
DISTINGUISHING FEATURES OF THE CLASS: The Case Manager is responsible for guiding, assisting, and linking older adults, and their caregivers, with appropriate supports and services. The work is performed both in the field and in the office through intake, in-home assessments, case management, development of care plans, assistance with forms, referral activities, and coordination of formal and informal support services.
MINIMUM QUALIFICATIONS:
A.Possession of a Bachelors degree in social work, gerontology, human services, or another related field; or B.Possession of an Associates degree in social work, gerontology, human services, or another related field and two (2) years of experience involving direct client contact in the delivery of services in a human services agency or program; or C.Four (4) years of experience as listed in (b) above.
NOTE: Verifiable part-time and/or volunteer experience may be prorated toward meeting full-time experience requirements.
