Northwest Regional Development Commission Minnesota Aging Pathways Resource Specialist in Warren, Minnesota
Job Duties
Specialists will provide long-term care options counseling by phone to help consumers understand service options and implement community resources that promote choice and independence. We utilize a person-centered model to link people to services that will help them remain in their homes and promote healthy aging.
Minimum Education Required
Minimum of a bachelor’s degree in social work, nursing, or a related degree.
Minimum Experience Required
Or minimum of 4 years’ experience in human services.
