Northwest Regional Development Commission Minnesota Aging Pathways Resource Coordinator in Warren, Minnesota
Job Duties
This position will be responsible for planning intensive care coordination for people wanting to remain in their home or relocate to the community after a stay in a care facility.
Minimum Education Required
Minimum of a bachelor’s degree from an accredited college or university in Nursing, Social Work, Gerontology, or a related human services field.
Minimum Experience Required
Experience working in person-centered discharge planning, charting, care coordination, or related tasks. Proficient in problem-solving skills, adept at phone etiquette, strong in computer skills, and have an independent work style.
