Position Data:
Title: Healthy Homes Program Specialist
Grade: 6 Exempt Non-Exempt $18.08/ hour Full -Time
Reports to: Healthy Homes Program Manager
Basic Function or Position Summary:
Navigate customers through the lead remediation process; communicate with customers,
landlords, contractors, and community partners to coordinate logistics for program
applications, open houses, lead inspections, and customer relocation.

Requirements
Position Responsibilities and Specific Duties:
• Serve as a liaison between COI and families
• Schedule and attend open houses for new lead remediation projects
• Serves as back up for reception.
• Schedule and attend lead clearance inspections
• Assist Healthy Homes Program Manager with communications with contractors
• Coordinate with Lead Program Clerk to establish customer eligibility
• Refer customers to other essential support services such as SNAP benefits, Medicaid,
emergency food, clothing, transportation, translation, health information and child
care
• Coordinate the relocation of families during the lead hazard control work in their
housing
• Recruit landlords, tenants, and homeowners for participation in the Lead Remediation
program
• Attend events to promote lead remediation services for Chautauqua Opportunities
• Conduct outreach and provide education to make the public (including tenants and
landlords) aware of lead poising hazards and prevention methods.
• Assist in the development of outreach materials, including but not limited to public
service announcements, posters, printed material, electronic media, and promotional
items
• Maintain agency documentation of services and customer progress.
• Promotes agency mission.
• Maintains confidentiality and complies with the code of ethics.
• Maintains consistent professional customer service.
• Performance incorporates integrated service delivery model while promoting self-
sufficiency.
• Other duties as designated by supervisor
Key Working Relationships:
A. Internal: Healthy Homes Program Manager, Healthy Program Clerk, Service staff,
Navigators
B. External: Service staff at community health organizations, Chautauqua County
Department of Health, landlords, customers, contractors
Supervisory Scope:
A. Number of staff supervised: 0
B. Titles supervised: N/A
Organizational Responsibilities:
• Adheres to all policies and procedures
• Gathers appropriate documentation and tracks outcomes
• Participates in organizational committee structures as appropriate
• Participates in organizational and divisional management systems
VIII. Knowledge, Skills Required:
• High school diploma; Associates degree in social or physical sciences preferred.
• Minimum two years of work experience in health, human, or customer services
required; customer service in a construction or trade field preferred
• Strong interpersonal, communication, engagement, and customer relations skills
• Strong attention to detail and follow up.
• Strong logistics, multi-tasking, and project management skills
• Ability to build trusting relationships with program participants and contractors.
• Ability to use and operate Microsoft office programs and Google applications
Physical Requirements:
• Ability to lift up to and including 10-25 pounds of physical effort
Special Requirements:
• Ability to obtain and maintain EPA RRP certification within six months of
employment
• Ability to obtain and maintain Lead Abatement Supervisor certification within one
year of employment
• Ability to obtain and maintain New York State notary license
• Ability to work occasional non-traditional hours, including evenings and weekends
• Must be able to provide consistent even tempered customer service at all times
• Valid driver’s license
• Reliable transportation
• Ability to travel throughout the county
• Works compassionately with a diverse population
• Experience navigating community programs
• Bilingual preferred, but not required
