Job Description:- Job Title – Behavioral Health Case Manager III Location- Georgetown, DE 19947 Duration – 12 Months Summary: Provides professional social work and behavioral health case management services in a psychiatric outpatient setting, including assessments, treatment planning, counseling, and risk management for individuals with mental health or developmental challenges. Collaborates with treatment teams, coordinates comprehensive patient care, and connects clients to appropriate community resources. Requires a Master’s degree in a Behavioral or Social Science field and at least three years of relevant case management and clinical experience. Job Description: Performs professional social work in a psychiatric outpatient setting by overseeing and participating in assisting the developmental disabilities, mental illness, or emotional disturbance to readjust and achieve a social, economic, emotional, and physical balance and personal independence compatible with generally accepted norms. Professional behavioral health case management services include but are not limited to assessments, treatment planning, direct service provision, person-centered services, and management of client risk and safety. Reports to an administrative superior and is responsible for providing comprehensive patient care services that support the integrity of the Department. Provides consultative services to other state and private agencies, as well as referring clients/patients to appropriate social service agencies. Minimum Qualification:
Possession of a master’s Degree in Behavioral or Social Science or related field.
Three years’ experience in case management, which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs using effective counseling or interviewing techniques, as well as other available resources such as service plans or case management systems.
Three years’ experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.
