Program(s): 830 - Supervised Residential: The SupervisedResidentialProgram provides adults living with substantial levels of mental illness or disability with 24/7 servicesthrough recovery-oriented communityresidentialfacilities.Southern Illinois Rural Healthcare Services Outreach program (SIRHSO) -SIRHSOfocuses on expanding access to preventative health care servicesin underserved rural areas through the following four national priority areas: Heart Disease, Cancer, Chronic Lower Respiratory Disease (CLRD), and Unintentional Injury. This consortium includes local healthcare providers and community-based partners who are committed to reducing health disparities and improving access to early detection and preventive services in our rural communities.

RN job duties include, but are not limited to:
Coordinate medication management across assigned programs, including pharmacy pickup and delivery, medication reconciliation, verification of accuracy, audits, refills, and proper disposal of discontinued or expired medications.
Maintain accurate EMAR and paper MAR systems and ensure readiness for system downtime, emergencies, off-site visits, and provider appointments.
Provide on-call nursing support and consultation to residential staff regarding emergent medical and medication-related concerns.
Monitor medication effectiveness, side effects, and changes in participant condition and communicate concernsto providers and leadership as appropriate.
Administer injectionsand provide direct nursing care within scopeof practice.
Monitor vital signs and chronic health indicators (e.g., blood pressure, BMI, HbA1c) for individuals with co-occurring behavioral and medical conditions.
Assist with psychiatric clinic coverage as needed, including obtaining vitals, supporting intake, performing medication reconciliation, tracking labs, and documenting nursing care in the EMR.
Coordinate and follow up on medical referrals, specialty care, and provider recommendations.
Schedule, track, and monitor medical appointments, attendance, and outcomes.
Review hospital and emergency departmentdischarge instructions and ensure care plan updates are accurately reflected in the EHR.
Support care staff by triaging clinical concerns, assisting with primary care linkage, requesting medical records, and reducing gaps in care.
Provide individualized and small-group health education on chronic disease self-management, medication understanding, symptom recognition, tobacco cessation, and appropriate use of urgent or emergency care.
Collaborate with dietitians and community partners to translate lab results and medical information into accessible, plain-language education for clients and care providers.
Participate in community outreach, mobile health, and health education events as assigned.
Support referral development and relationship-building with community partners and healthcare providers.
Assist with documentation, data collection, performance measures, and quality improvement activities required for grant-funded programs.
Maintain compliance with agency policies, nursing standards, and regulatory requirements.
Ensure accurate and timely service documentation and participate in audits, surveys, and quality improvement initiatives.
Maintain current licensure, certifications, and required trainings.
Communicate professionally with staff, providers,leadership, and external agencies.
Support continuity of care across residential, outpatient, hospital, and community settings.
Provide nursing coverage across agency programs as emergency needs arise.
