PeaceHealth Director Hospice Care in Vancouver, Washington

PeaceHealth is seeking a Director Hospice Care for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $67.74 – $101.60. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Job Summary

Accountable for hospice program development and optimization across PeaceHealth. In collaboration with the Home & Community Leadership Team (HCLT), will develop long and short-term business and payer strategies, operational plans and supports implementation of key operational, clinical, and business strategies supporting the hospice services line. Provides leadership and oversight to hospice strategic development and implementation of processes, in partnership with the HCLT, clinical and non-clinical leadership, and system/network leaders. The Director Hospice Care must ensure that quality and performance improvement activities meet their target conditions. The Director is responsible to adhere to all applicable federal, state, and local laws, regulations, and standards for their areas and to be continuously ready for any survey on those regulations or standards.

Essential Functions

  • In partnership with HCLT, establishes monthly, quarterly, and annual program quality, operational and growth goals.

  • Leads efforts to achieve and/or exceed determined goals for quality, operations, and program growth.

  • Ensures program compliance with all organizational, state, and federal regulations and organizational policies and procedures.

  • Maintains partnerships with key stakeholders such as leadership, project sponsors, etc. at the system, network, and community level.

  • In partnership with HCLT and key stakeholders, establishes system-wide goals and metrics for hospice care. Monitors goals and metrics and makes improvement recommendations to the communities.

  • Takes responsibility for the coordination and management of data. Tracks hospice care data, analyzes trends and identifies opportunities for improvement. Ensures reporting on findings to key stakeholders.

  • Leads and participates in monthly hospice care leadership team(s) conference calls ensures monthly reports of project progress as well as presenting outcome metrics are provided. Participates on hospice care industry leadership collaborations.

  • Develops presentations and written communications; partnering with HCLT, hospice medical directors and other key stakeholders at all levels of the organization to champion hospice care strategic plan(s).

  • Facilitates teams for related hospice care project work and supports other teams’ work performed in relation to hospice care outcomes.

  • Collaborates with contracting to ensure appropriate reimbursement is obtained for services.

  • Performs other duties as assigned.

Qualifications

Education

  • Bachelor's Degree Required: Healthcare or related field and

  • Master's Degree Preferred: in Healthcare or business or a combination of education and professional experience that provides the caregiver with the requisite knowledge, skills, and abilities to successfully perform the job

Experience

  • Minimum of 7 years Required: direct patient care experience in a healthcare setting; preferably, community-based care, hospital, clinic, or health system related and

  • Minimum of 5 years Required: management experience in a healthcare setting; preferably community-based care or health system related and

  • Required: Experience with managing complex programs and projects, specifically with a focus on customer service/service excellence/patient experience activities and demonstrated successful completion of quality improvement projects with desired outcomes

  • Working knowledge of Hospice, Home Health and/ or Palliative care as well as general analysis tools and concepts (validity, reliability, etc.) (Required)

  • Working knowledge of Microsoft tools. (Required)

Working Conditions

Lifting

  • Consistently operates computer and other office equipment.

  • Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.

  • Sedentary work.

Environmental Conditions

  • Predominantly operates in an office environment.

Mental/Visual

  • Ability to communicate and exchange accurate information.

  • The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
