Position Description

We are currently seeking a Registered Nurse to join our POGO Interlink Nursing Team in the Division of Haematology/Oncology. As a vital member of the care team, you will provide family-centered nursing care to children and their families along the continuum of care with an emphasis on the transitions between hospital and community.

This role requires travel to community settings and on-site work at SickKids. The planned catchment areas are Scarborough and Durham Region. Assigned areas may change over time to adapt to changes in service delivery.

Here's What You'll Get To Do

  • Navigate the complex healthcare system and act as a resource and support to families and children with an oncology diagnosis

  • Extend the care of patients and families from hospital to home and community

  • Provide patient and family education and link families to appropriate resources

  • Conduct home visits to continue assessments, initiate services and support patients and families related to the impact of a cancer diagnosis, including palliative care

  • Provide support to siblings, extended family, teachers and principals around a patient's illness and treatment plan

  • Provide bereavement support to family, community and school as appropriate

  • Maintain ongoing communication with the primary and multi-disciplinary teams

  • Provide education to our community partners around pediatric cancer diagnoses, treatment and impact to families

  • Participate in POGO Interlink Nursing Team meetings and educational sessions

Here's What You'll Need

Essential

  • Baccalaureate degree in Nursing

  • Registration with the College of Nurses of Ontario as a Registered Nurse in good standing

  • Minimum of 5 years of experience in Paediatric Oncology Nursing

  • Demonstrated commitment and actions in advancing equity, diversity, and inclusion objectives.

  • A valid driver's license in Ontario and access to a vehicle, with the ability to travel to community settings for home and school visits within the service area

Assets

  • APHON Chemotherapy and Biotherapy certification

  • Excellent interpersonal and communication skills

  • Ability to be a self-learner and be self-directed

  • Ability to work within an independent practice model
