JOB REQUIREMENTS: Community Horizons is assisting a member to find a caregiver to provide support that will help ensure quality of life while remaining in her home. She is a kind person who enjoys knitting, visiting, and attending church. This is a self-directed care roll. You can help have a huge impact on her life by providing short visits 4 times/week.
