What You’ll Do
Deliver high-quality, in-home physical therapy services aligned with Elara Caring’s mission and standards
Evaluate patient functional needs, establish measurable goals, and adjust care plans to achieve optimal outcomes
Design and implement individualized plans of care with patient and caregiver involvement
Collaborate with interdisciplinary team members to support patient progress
Provide direction to support staff to help patients meet functional goals
Maintain accurate, timely documentation including evaluations, treatment plans, and progress notes
Serve as a clinical resource and advocate for physical therapy within the community
What You’ll Need to Be Successful
Graduate of an APTA-approved Physical Therapy program
Current, unrestricted Physical Therapist license in the state of practice
Minimum one (1) year of experience in a healthcare setting
Ability and willingness to travel within the assigned coverage area (up to 50%)
Reliable transportation, valid driver’s license, and current auto insurance
Ability to sit, stand, bend, lift, and move intermittently; must be able to lift 50–100 lbs
