Are you passionate about supporting Veterans and helping them live independently at home? Western Montana Aging Services is seeking a Veteran Directed Care (VDC) Service Coordinator to serve as the primary liaison between the VA Medical Center and Veterans participating in the VDC program in Western Washington.
In this full-time role, you will:
- Provide person-centered counseling to Veterans and caregivers.
- Assist Veterans with self-assessments, service planning, and hiring caregivers.
- Conduct monthly phone check-ins and quarterly in-person visits.
- Collaborate with VA staff and community partners to support Veteran independence.
- Ensure accurate documentation and program compliance.
Qualifications:
Bachelor's degree in Social Work, Human Services, Nursing, or related field preferred; equivalent education/experience considered. Experience with Veterans, case management, or the VA system is highly desirable. Veterans and individuals with a history of serving Veterans are encouraged to apply.
Compensation & Benefits:
- Wage: $25--$30 per hour, full-time, primarily day shift Monday--Friday (some weekends and overnight travel required; schedule flexibility to meet Veteran needs).

- Requirements include a valid drivers' license and dependable transportation, proof of insurance, ability to pass a background check.
