Eagles Adult Day Inc. Therapeutic Recreation Program Coordinator in Staten Island, New York
Plan, organize, direct, and participate in therapeutic programs to facilitate patients' rehabilitation. Conduct assessments and create individual treatment plans to meet clients needs based on needs assessment, interests, and objectives of therapy. Instruct patients in physical activities and techniques to improve mobility, strength and balance or alleviate pain. Observe, analyze, and record patients' participation, reactions, and progress during treatment sessions, modifying treatment programs as needed. Obtain information from medical records, medical staff, family members and the patients themselves, to assess patients' capabilities, needs and interests. Confer with members of treatment team to plan and evaluate therapy programs. Encourage clients with special needs and circumstances to acquire new skills and get involved in health-promoting therapeutic activities. Prepare and submit reports and charts to treatment team to reflect patients' reactions and evidence of progress or regression.
Requirements: Masters Degree in rehabilitation, physical therapy, rehabilitation counseling, social work + 3 months of job experience.
