The Disability Support Specialist will plan, implement, and monitor response to information access needs of programs and services administered by the Aging and Disability Services Administration through the Disability Services Division. Responsibilities include:

  • Provide a coordinated customer service and technical assistance response to external and internal inquiries generated by the Disability Hub MN and Disability Services Division contact/response systems

  • Support related training, communication, outreach, and systems management activities to meet project/program goals for Disability Hub MN and Disability Services Division

  • Manage and respond to information and assistance requests from the Disability Hub MN system and Disability Services Division Response Center inquiries

  • Support the information, documentation and maintenance of the Disability Hub MN training opportunities and modules

  • Participate with division and Hub team(s) to develop, implement and monitor protocols, for providing Disability Hub MN andDisability Response Team services, to DHS, other state agencies, and providers, to support the projects and the Division’s strategic goals

Minimum Qualifications

2 years professional experience in human services programs providing information, resources, referrals and/or technical assistance.

Experience must demonstrate the following:

  • Understanding of State and Federal Health, Home and Community Based, and continuing care programs

  • Program knowledge of Medical Assistance, General Assistance Medicare, Managed Care, and MinnesotaCare

  • Customer service and technical assistance skills for providing relevant information, assistance and referrals

  • Knowledge of disability and employment policy, practices, providers and organizations

  • Proficient in MMIS, MAXIS, and other information platforms, such as METS, SMI/SOLQ, CRM and Revation

  • Skilled at using MS Office Suite programs: Word, Excel, PowerPoint, Outlook, Teams, and SharePoint

Bachelor’s degree in Human Services, Public Policy, Social Work or related field may substitute for 1 year of professional experience OR Master’s degree in Human Services, Public Policy, Social Work or related field may substitute for 2 years of experience

Preferred Qualifications

  • Trained in person-centered thinking and planning

  • Experience managing eligibility for programs or managing waiver service access

  • Contact center experience

  • Assisting people with navigating the complex human service system

  • Plain language and accessibility experience

  • Organize work, anticipate problems, monitor performance, set priorities and meet deadlines

  • Strong human relations skills and ability to communicate well with a variety of clientele

  • Ability to facilitate discussions to guide, solicit and explore diverse perspectives

  • Variety of experiences working effectively with others from different backgrounds and/or cultures
