Registered Nurse / Case Management Home Hemodialysis and Peritoneal Dialysis (HHD and PD)
Full time hours Monday-Friday generally 8am-5pm
no weekends or holidays
must have at least 9 months of post-graduate RN experience
prior Case Management exp. is desired but not required
LOCATION:
Incoming RN will work primarily in St. George but needs to travel to Hurricane, Mesquite, and Cedar City to speak with patients as needed.
TRAINING TRAVEL REQUIREMENT:
Majority of training for this role will be done in Utah County with an experienced preceptor. Incoming teammate must be willing to travel to this area during the first 3 months for up to 1 week at a time.
DaVita is seeking an organized, self-directed RN to manage Home Hemodialysis (HHD) and Peritoneal Dialysis (PD) patients. You'll educate patients and caregivers in-clinic, enabling them to perform dialysis independently at home. This role blends clinical care, patient education, and case management.
Key Responsibilities:
Train patients and caregivers on HHD procedures and self-care
Manage a caseload of HHD patients, including assessments and care planning
Monitor treatment progress, conduct home visits, and provide on-call support
Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
Schedule:
Monday-Friday, 8-hour shifts
Weekends and holidays off
Flexible scheduling to manage your own caseload
Requirements:
RN license in Utah, and if already licensed in Nevada as well that is a plus.
Must have 9 months of post-graduate RN experience for this HHD/PD role
Valid driver's license, insurance, and reliable transportation
ADN required; BSN preferred
Basic computer skills (MS Word, Outlook)
Preferred Experience:
Dialysis (HHD, PD, chronic, or acute)
Patient education or teaching background
Case management or telehealth experience
CNN or CDN certification
