Salary Range: $75,000 – $80,000

What You’ll Do
Provide compassionate hospice nursing care to patients and families
Educate patients and caregivers on the hospice process and plan of care
Guide families through goals-of-care discussions and healthcare decisions
Conduct initial and ongoing comprehensive assessments
Perform hospice evaluations and informational visits
Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members
Coordinate care to ensure patient and family needs are fully met
Communicate changes in patient condition and update plans of care as needed
What You’ll Need
Active Missouri or Compact RN license
Hospice or Long-Term Care nursing experience preferred
Strong assessment, communication, and care coordination skills
Valid driver’s license and auto liability insurance
