MAJOR DUTIES & RESPONSIBILITIES:

[The Pharmacist will be reporting to the Pharmacy Director and will be working closely with a team of tenured pharmacy technicians. Independently reviews and assesses medication orders to identify, prevent, and resolve potential and actual drug-related problems. Contacts physicians, nurses and other professionals as necessary to facilitate care. Manages drug therapy protocols as part of physician-pharmacist-nurse team.]{times="" new="" roman",="" times,="" serif"=""}
·[ Performs specialized order entry functions and maintains pharmaceutical care documentation.]{times="" new="" roman",="" times,="" serif"=""}
·[ Responds to questions from physicians and nursing staff and patients regarding medicines and pharmaceuticals.]{times="" new="" roman",="" times,="" serif"=""}
·[ Monitors individual patient drug therapy for medication safety and appropriateness.]{times="" new="" roman",="" times,="" serif"=""}
·[ Monitors and updates charge system for drugs and pharmaceutical services.]{times="" new="" roman",="" times,="" serif"=""}
·[ Documents drug therapy clinical interventions, medication errors, and adverse drug reaction events per pharmacy operational protocol.]{times="" new="" roman",="" times,="" serif"=""}

QUALIFICATIONS:
