Position Purpose

The Community Health Worker (CHW) is a trusted member of the community who serves as a bridge between individuals, health and social services, and food security resources. In this role, the CHW supports community members (predominately Spanish speaking) in accessing nutritious food and related services, builds relationships through outreach, and provides culturally responsive health education on topics such as healthy eating, urban gardening, and navigating food assistance programs. The CHW uses motivational interviewing and behavior change strategies to help participants build skills, improve health behaviors, and strengthen self-sufficiency. An interest in nutrition, gardening, and agriculture preferred, with flexibility for occasional evening and weekend hours.

This position supports the Food Security Coordinator with key programs—including the Community Garden and Greenhouse, the Choice Food Pantry, the Farmer’s Market, and health literacy initiatives related to nutrition and food access. The CHW assists with programming, participant engagement, resource navigation, and community-based activities that enhance food security and promote equitable access to healthy food.

As a “Community Health Worker” you will:

  • Delivers services to vulnerable and high-risk individuals, particularly those hard-to-reach and/or with social needs, in community-based settings.

  • Build relationships within the community (i.e., faith organizations, community groups/centers, community leaders, community businesses, etc.)

  • Reinforces health literacy and educational messages using appropriate tools and supplies within scope of practice.

  • Support food security events & programs, including outreach, event preparation and on-site assistance, sharing resources, and basic data and recordkeeping.

  • Assist with launch of new food pantry, including check-in logistics, stocking and inventory, volunteer coordination, and supporting community partnerships.

  • Assist with farmers’ market operations at Montgomery College, Germantown.

Minimum Qualifications:

  • Comprehensive knowledge of the local community.

  • High school graduate or GED required.

  • Certified Community Health Worker, preferred.

  • Minimum of two years' community involvement experience (i.e., community health worker, volunteer for community-based organization, outreach worker, community navigator, etc.)

  • Ability to effectively present information in one-on-one and small group situations to clients, and other employees of the organization.

  • Ability to communicate verbally and in writing and to hear or communicate with target population, medical and social service professionals, colleagues, and peers.

  • Ability to inspire and motive others.

  • Motivational interviewing skills, preferred.

  • Proficient with Microsoft Office applications including Word, PowerPoint and Excel

  • Bi-lingual skills (Spanish) to work with patients in their native language are preferred.

Pay Range: $22.83 - $31.96

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

0.5 FTE (20 hours weekly) up to 1.0 FTE (40 hours weekly)**
