Job Description:ESSENTIAL JOB FUNCTIONS- Make appointments and/or referrals for PCP or Medical Specialty. - Maintain the Integrated Care Coordinators schedule including appointment reminders, scheduling appointments and rescheduling following a cancellation or no show.- Prepares and mails caring contacts to clients determined to be at a risk of suicide as directed by Integrated Care Coordinator.- Assists clients in navigating the healthcare environment to ensure appropriate access to care.- Assists with coordination/access to services that address social determinants of healthcare outcomes: - Assist with options for transportation to healthcare appointments - Assist client in obtaining housing resources - Assists with access to other community resources e.g., public assistance and financial benefits- Develops knowledge base of other healthcare providers in the service area and collaborate, as needed- Documents tracking for appointments, medication, and other healthcare needs for assigned clients, as neededEDUCATION- High School diploma or GED- Preferred, but not required training as a Medical AssistantEXPERIENCE- One year of experience working in a hospital, physician office, or behavioral care clinic- Experience in community setting or integrated care environment.PHYSICAL DEMANDS- Position may require lifting of 20 pounds maximum, with lifting or carrying of items weighing up to 10 pounds, or requires significant periods of walking or standing- Normal tasks may involve exposure to blood or bodily fluids- Reliable transportation for travel between clinic locations if needed.Time Type:**Full timeSeven Counties Services is an equal opportunity employer.
