The Youth Connection Coordinated Entry & Data Quality Specialist in Shelton, Washington

Reports To: Lead Case Manager / Program Manager Pay Range: $21.00 to $25.00 Status: ? Full-Time ? Part-Time

 

The Youth Connection (TYC) serves youth and young adults ages 12--24 who are experiencing homelessness or housing instability. TYC works to prevent and end youth homelessness by providing low-barrier, Housing First--aligned, housing-focused services---including Coordinated Entry (CE), case management, Rapid Re-Housing, Eviction Prevention, and supportive services---that prioritize safety, equity, and long-term housing stability through strong community partnerships.

The Youth Connection was founded in 2018 as Shelton Family Center with the Vision of providing a safe place for all youth to connect, learn and grow in the community. Our mission is to make connections and change the lives of the youth 12- to 24-year-old. Over time our focus is still to provide a safe place for youth to hang out, make connections and ensure all young people have a safe place to sleep every night.

PURPOSE OF THE POSITION

The Coordinated Entry & HMIS Data Quality Specialist is responsible for implementing Coordinated Entry and HMIS requirements in compliance with Commerce/CHG and HUD standards.

This position ensures:

  • Standardized CE assessments are completed consistently for youth
  • HMIS data is accurate, complete, timely, and audit-ready
  • The Youth By-Name List is maintained in alignment with CE prioritization rules
  • Data quality supports referrals, reporting, and contract compliance

This is a compliance-critical role tied directly to CHG performance, monitoring, and reimbursement.

 

STANDARD OPERATING AUTHORITY

When local or county guidance conflicts with HUD or Washington State Department of Commerce / CHG requirements, The Youth Connection follows HUD and Commerce/CHG guidance. County procedures are implemented only when they do not contradict higher-authority requirements.

 

PRIMARY RESPONSIBILITIES

Coordinated Entry (CE) -- Primary

  • Conduct and support standardized Coordinated Entry assessments for youth and young adults using the approved youth assessment tool.
  • Ensure all youth are assessed consistently and without bias, regardless of funding source or program interest.
  • Maintain the Youth By-Name List, ensuring status, prioritization, and updates are entered promptly following changes.
  • Prepare CE data and documentation for By-Name / case conferencing meetings in alignment with Commerce CE guidance.
  • Support CE referral processes by ensuring data accuracy and documenting eligibility without re-ranking or altering prioritization.
  • Track follow-up timelines and document outreach attempts for youth on the By-Name List, flagging inactivity thresholds per CE policy.

This position does not have authority to alter CE prioritization, referral order, or eligibility determinations and must escalate conflicts to supervisory staff.

 

HMIS & Data Quality -- Primary

Enter and update HMIS data for Coordinated Entry, Care Coordination, and Diversion activities within required timelines, consistent with the HMIS Data Quality Plan.

Review new intake packets for completeness, consent, and required approvals prior to HMIS entry.

Ensure Current Living Situations and exits are entered promptly upon receipt.
