SEAVIEW COMMUNITY SERVICES ACT Case Manager in Seward, Alaska

About SeaView

SeaView Community Services has been serving the Seward community since 1972, providing a full continuum of behavioral health, housing, and supportive services. Our Assertive Community Treatment (ACT) program is the largest outpatient program at SeaView and plays a vital role in delivering intensive, community-based care to individuals with serious and persistent mental illness.

We are seeking a compassionate and dedicatedACT Case Manager to join our team. This position offers the opportunity to make a direct impact in the lives of clients by supporting recovery, independence, and improved quality of life.


Responsibilities
  • Provide intensive case management services as part of a multidisciplinary ACT team.

  • Assist clients with accessing housing, employment, medical care, benefits, and community resources.

  • Conduct home visits, community-based support, and outreach to ensure continuity of care.

  • Support clients in achieving goals around daily living skills, wellness, and recovery.

  • Document services in compliance with Medicaid and program standards.

  • Collaborate with team leader, clinical associate, nurse, and psychiatrist to provide wraparound, person-centered care.

  • Participate in weekly team meetings, crisis response, and 24/7 coverage rotation as assigned.


Qualifications
  • Bachelor's degree in Social Work, Psychology, Human Services, or related field (or equivalent experience) required.

  • Previous experience working in behavioral health, case management, or social services strongly preferred.

  • Ability to work independently and as part of a collaborative team.

  • Strong communication, organization, and problem-solving skills.

  • Valid driver's license and ability to travel within the community, to conduct company business.

  • Must be able to pass background check and meet Alaska Medicaid compliance requirements.
