Providence St. Joseph Health Program Support Assistant - Case Management in Seattle, Washington

Description Join a collaborative and missiondriven Case Management Team (CMT) dedicated to supporting patients throughout their care journey. As a Case Management Associate, you'll work closely with nurse case managers and social workers to help ensure patients receive the right care in the right setting. This role is ideal for someone who is organized, compassionate, and thrives in a fastpaced healthcare environment where teamwork and communication make a meaningful impact. Key Responsibilities: Participate as an active member of the Case Management Team (CMT). Under the guidance of the nurse case manager and social worker, assist in facilitating appropriate patient placement. Utilize hospital and community resources to support continuity of patient care. Serve as a Utilization Management liaison. Collaborate effectively with Medical Staff, CMT members, the Business Office, Medical Records, and other hospital departments. Required Qualifications: 1 year of job-related experience in a healthcare setting Preferred Qualifications: Associate's Degree Bachelor's Degree Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology upon hire Prior experience in discharge planning, billing, utilization review, financial counseling or registration Proficiency in medical terminology Why Join Providence Swedish? The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
