Requirements:
- Minimum of 1 year of experience in a senior care or related healthcare setting
- Current CPR certification
- Authorized to work in the US
- Must successfully pass a criminal background check and any additional screenings as required
- Two verifiable professional references
- Must maintain all required certifications, training, and comply with company and state regulations for employment
- Reliable transportation to attend assigned patient visits
Main Responsibilities include but are not limited to:
- Assisting clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) according to the Plan of Care
- Building trusting and confidential relationships with clients and their families
- Maintaining a clean, safe, and comfortable environment for the client
- Providing support with household tasks, personal care, and preparation of nutritious meals
- Assisting with therapy exercises as directed in the Plan of Care
- Monitoring and supporting healthcare needs, including providing medication reminders
- Encouraging clients to maintain or regain independence whenever possible
- Promoting mental stimulation through engagement in activities and conversation
- Accurately and promptly documenting client activities, mood, challenges, and other relevant information
- Reporting any changes in client condition or home environment to the immediate supervisor
- Attending in-service training sessions as required by state and federal regulations
