Schenectady County Department of Social Services Caseworker Trainee in SCHENECTADY, New York
Assess, formulate, and carry out plans to meet the needs of the individual or family. Periodically review effectiveness of plans to monitor safety and ensure that basic needs are met. With respect to individuals rights, motivate the individual(s) to increase their own capacity and confidence in their ability to address areas of concern. Review available background information, including identified needs for individuals referred, gathering information from the child(ren) or adult(s) themselves, their natural supports including household members, relatives, neighbors, community supports and service providers. When a safety plan requires a change of living arrangements such as foster care (children), assisted living/nursing facility (adults) or other institutional setting, determine the least restrictive or most appropriate setting to meet the persons needs. Plan, together with parents, caregivers, and relatives for the care of the child(ren) or family member to re-establish and stabilize the individuals living environment. Build professional relationships with individuals and family members to provide them with support and guidance to access medical, social, psychological, psychiatric and other recommended services. Identify the need for services by gathering information during in-depth interviews with individuals and families. Maintain professional relationships with community agencies to which individuals and families can be referred for services. Work closely with LDSS staff in carrying out the plan for services. Periodically review plans to identify changes in the individual or familys need for services. Use computers to maintain case records and correspondence. Remain flexible to the needs of the Agency and accept additional tasks, e.g., on-call, as assigned.
