Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
The opportunity:
We are currently looking to fill One Full Time, Permanent Supervisor role in our North East region – Sault Ste. Marie, ON
- Hours: 35 hours per week.
Responsibilities:
The Supervisor oversees programs which provide Community Support Services to clients with disabilities and seniors. These services include the provision of personal care and activities of daily living to support clients to remain independent in their home environment.
The Supervisor will often work with community partners so a focus on collaboration, teamwork and flexibility is required.
The Supervisor is responsible for overseeing service delivery, administration and supporting the management of human resources to meet client’s needs within MODC’s policies and procedures.
Will complete client intakes and provide client orientation, care planning and ensure that service contracts are accurate and done in a timely manner.
Will complete regular assessments of client needs within their own home environment within a timely manner.
Will support client scheduling and coordinate staff using internal scheduling systems.
A team approach to problem solving is required, as well direct contact with clients, staff, community partners and MODC management team. Building professional relationships with collaborative partners, community support agencies and hospital discharge planners is essential for arranging coordination of services.
Performs duties in a professional manner, which respects the dignity and right to privacy and maintains client and staff health and safety.
Must be flexible and demonstrate initiative to assist in all service delivery programs with waitlist management and operations.
Travel throughout the Sault Ste. Marie and Algoma is required to complete intakes, assessments, and service delivery support.
Typically scheduled to work regular business hours however MODC provides 24-hour support to its clients and the Supervisor will oversee staff who work various shifts.
To participate in service delivery and on-call responsibilities, and other duties as assigned.
Qualifications and experience:
Degree or Diploma in Human Services/Healthcare education. Preferred background in Nursing is considered an asset.
2 – 5 years experience coordinating services for Seniors or people with disabilities highly desirable
Community-based experience working with seniors or individuals with a disability
Highly organized and excellent time management skills
Excellent English communication skills, written and verbal
French-English bilingualism an asset
Demonstrated ability to supervise and train employees including organizing, prioritizing and scheduling work assignments
Problem solving and conflict resolution skills
Strong observation and assessment skills
Knowledge of behavior management principles
Current Standard First Aid and CPR certification
Valid driver’s license and own reliable vehicle is required.
Salary: $56,632
