Pacific Health Group is hiring a Lead Case Manager to support individuals with complex medical, behavioral, and social needs through our Enhanced Care Management programs. This role is ideal for someone who is passionate about whole-person care and building trusted, long-term relationships with members.
As a Lead Case Manager, you will work remotely while conducting local, in-person visits throughout Santa Clara County. You will assess member needs, coordinate medical and community-based services, and advocate on behalf of individuals facing challenges related to healthcare access, housing, food insecurity, and behavioral health. You will serve as a consistent point of support and help ensure members receive timely, coordinated, and compassionate care.
