Attend to hygienic care of clients. Bathe clients. Dress and undress clients. Change soiled diapers to clean ones as needed by clients. Provide oral care. Assist in toilet use. Transport clients from bed to wheelchair as needed. Change linens and make beds at least once a day or as needed. Serve meals and nourishment to clients as directed by supervisor depending on client's diet. Prepare clients for sleeping through hygienic and oral care. Report any unusual change of physical condition to charge nurse or administrator.
