Job Summary and Responsibilities
As our Occupational Therapist, you will provide essential in-home occupational therapy services under physician's orders. Your role involves evaluating home safety, assessing functional needs, and implementing tailored treatment to promote independence.
Every day you will evaluate home safety, assess functional needs of physically disabled patients, then develop, plan, and implement individualized occupational therapy treatment procedures.
To be successful, you will demonstrate strong assessment, treatment planning, and implementation skills for in-home care. Excellent communication for patient education and a commitment to promoting independence and safety are essential.

Performs patient evaluations appropriately and competently. Coordinates with the physician to establish a reasonable and necessary treatment plan.
Demonstrates compentence in the use of equipment in the area of assignment. Implements age-appropriate safeguards.
Plans and performs treament procedures appropriately and competently.
Updates, as needed, the treatment/rehab program, goals, and objectives in collaboration with team members.
Promotes the discharge planning process.
