St. James Zion House Case Manager (Homeless Veteran Reintegration Program - Veterans Employment Assistance Services (HVRP VEAS) in Salisbury, Maryland

HVRP-VEAS is a grant funded program at St. James AME Zion Church - Zion House through the U. S. Department of Labor intended to assist homeless Veterans on the Maryland, Virginia and Delaware Eastern Shore to obtain gainful employment. The HVRP-VEAS team will work closely with community partners to identify eligible veterans in need of employment. Through provision of case management services, Veterans will be assisted in navigating the employment search and placement system. The goal of the HVRP-VEAS program is to assist clients to obtain and retain employment necessary to maintain long term, independent living. HVRP-VEAS is an intervention program, with services to include case management and assistance in obtaining employment for Veterans.

Position The Case Manager reports to the Supervisory Case Manager and complements the HVRP-VEAS team of Zion. The primary purpose is to help Veterans obtain and retain gainful employment. This position is critically important to the success of the HVRP-VEAS project and requires a high degree of collaboration and public relations skills.

Responsibilities: The HVRP-VEAS Case Manager duties are summarized as: Responsible for the development of emergency support systems to meet client needs by identifying and coordinating a variety of available services necessary to obtain training, retooling, and to obtain and retain employment.

? Provide direct supportive comprehensive case management services to homeless veterans. ? Conducts crisis intervention and provide referrals as necessary ? Screens and evaluates veteran households for housing needs and eligibility for HVRP-VEAS services ? Provide intake and assessment services ? Develop individual employment plans based on the desires of the Veterans served ? Collect necessary data, determine program eligibility and create participant case file ? Maintain and builds partnerships and coalitions with other community and government agencies ? Participates and assists with outreach events ? Counsel participants to support them in reaching their goals ? Track and provide program outcomes to the Supervisory Case Manager to update the TPR for reporting to the DOL monthly, assist with compiling and submitting required reports.

? Participates in discussions and action to resolve case-specific situations regarding client progress. ? Coordinate care with area agencies and community partners on behalf of participants for eligibility documentation, financial assistance, and employment training services. ? Maintain participant files and ensure that all required documentation is contained in each file ? Identifies and locates employment options suitable for veterans, as needed

Requirements: ? Bachelor's degree in Human Services, related field or equivalent work experience. Masters degree preferred. ? 1 - 3 years of experience working with veterans, people in recovery, low income families and or homeless individuals ? Excellent oral and written communication skills ? Proficiency with Microsoft Office and HMIS ? Ability to work both independently and as a member of a team ? Valid Maryland driver's license, insurance and reliable transportation ? Must be proficient in record keeping, tracking, maintaining, and reporting according to program guidelines and requirements ? Public speaking and presentation skills ? U.S. Veteran or Veteran spouse strongly preferred. ? Able to lift 25 pounds ? May be required to sit at a computer station for extended periods of time to complete essential job functions.
