FIVE COUNTY ASSN OF GOVTS CASE MANAGER - AGING in SAINT GEORGE, Utah
JOB DESCRIPTION:
We're seeking a compassionate, organized Case Manager to support older adults and strengthen community-based services across the Five County Region. The position provides professional case management services for the Aging Waiver Medicaid, Alternatives, and Caregiver Support Programs. Key responsibilities include intake, screening, eligibility determination, assessment, care plan development, service coordination, and maintenance of client records. Services are delivered in community settings such as clients' homes, caregiver residences, assisted living facilities, or other approved locations. The position also supports information and referral services, outreach activities, community events, and training.
EDUCATION AND EXPERIENCE:
A bachelor's degree in social work or a related human services field, plus two years of case management experience. Valid driver's license and ability to travel throughout the Five County service area. Social Service Worker (SSW) license or certification through the National Academy of Certified Care Managers required within 6 months of employment. Experience in long-term care services and support preferred.
Candidates must demonstrate strong written and verbal communication skills and a solid understanding of assessment, care planning, and care coordination. Preference will be given to applicants with SSW license or approved case management certification and those with direct case management experience, familiarity with longterm care services, or knowledge of human service resources within the region. Proficiency with Word, Excel, and webbased software is essential. Applicants must be able to interact professionally with staff, community partners, and the public, and possess general office and phone etiquette skills.
WORK SCHEDULE:
Fulltime, 40 hours per week (Monday--Friday) with flexible scheduling options
WORK-SITE:
St. George or Hurricane office. Remote work may be possible; however, employees must reside within the Five County service area and be available for home visits, inperson meetings, trainings, and periodic onsite work as required.
