Purpose Supported Living Services LLC Direct Care Professional in Sacramento, California
Purpose SLS Job Description Classification Title: Direct Care staff Definition, Staff who personally provide direct service to Consumers. Personnel who provide hands-on care, support, and assistance to individuals and consumers needing services. Directly responsible to Customer In a home setting.
Responsibilities and Duties:
Ensure the consumer's security and maintain a safe environment.
Encourage self-help activities. Foster positive relationships among direct care staff , individual, and the consumers they serve. Perform home management functions such as Lighthouse keeping, laundry, bed making, and cleaning. Plans meals, shopping for groceries, preparing and serving meals. Performs Personal Care tasks, including assistance with basic personal hygiene and grooming, feeding, ambulation, medication monitoring, and minor healthcare-related tasks. Reminds and assists consumers with self-administration of medication with proper training. Documents Services provided. Accompanies consumers to scheduled appointments. Support consumers with errands. Ability to handle physical aspects of the job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time. Ability to work in a home-like setting.
Minimum requirements Must possess a diploma, GED, or equivalent, perform first aid and cardiopulmonary resuscitation, and operate 24-hour emergency assistance systems. Current and valid license certificates or registrations that may be legally required to perform the services. Skills and Abilities The skills, training, and education necessary to establish and maintain a constructive and appropriate personal relationship with consumers and minimize the risk of endangerment to the health, safety, and well-being of consumers. To record Community activities properly and perform excellent documentation of daily living activities (DLA). Excellent Reports, Communication, and Writing skills.
