Job Description:
Home Visit Practice
- See 2-4 patients in each half-day care session in the community (generally patient’s own home but may be in other private dwelling including comfort care home or a family member/caregiver’s home). 2 sessions per week depending upon need.
Office Practice
See 4-8 patients in each half-day care session. 6 – 4 hour half day sessions per week depending upon need.
These sessions are generally at GAMA
1-2 half day sessions may be done from home via telemedicine dependent on clinical availability, personal preference, and insurance guidelines for telemedicine coverage
Office Practice (Non-Face to Face Time)
Assist office nursing and secretarial staff in phone management and triage of patients.
In basket management of provider’s personal in basket and those of any other provider who is unavailable
Complete clinical documentation, forms/paperwork requested by patients, prior authorization appeals, sign home care orders
Coordinate with other providers, home care and hospice teams, and pharmacies
Assist in development of practice, community outreach, educational materials, ect…. Duties may include presentations
Maintain professional credentials to allow for uninterrupted billing and clinical activity.
Provide inpatient palliative care coverage as needed for vacations, holidays, and any prolonged leaves
Salary Range:
$70.00 an hour for Day Shift
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Job Requirements:
Education: Fulfills the requirements to be licensed as a Nurse Practitioner in NYS
Experience: Home care, hospice/palliative care, inpatient care, nursing home, geriatrics preferred
License/Certification Required: Valid New York State license as a Nurse Practitioner and current DEA registration
Skills: Demonstrated excellence in written and spoken communication, particularly with complex chronic medical disease, end of life issues, and complicated family systems.
