Description SUMMARY As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. Job Description JOB TITLE: Certified Home Health Aide (HHA), Home Care (Per Diem) DEPARTMENT: Home Care LOCATION: Rochester Regional Health Home Care - 330 Monroe AvenueRochester, NY 14607 SCHEDULE: Per Diem ATTRIBUTES Compassionate, patient focused and dependable professionalCritical thinking skills, decisive judgment and the ability to work with minimal supervisionExceptional documentation and communication skills Ability to read, write and speak English, ability to follow verbal and written instructionsRESPONSIBILITIES Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly Documentation. Accurately and completely document client care on activity sheet Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirementsPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. LICENSES/CERTIFICATIONS: HHA - Home Health Aide certification - New York State Education Department (NYSED)NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)PAY RANGE: $18.65 - $19.75The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
