Job Summary and Qualifications

The Social Worker Case Manager PRN is responsible for promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified medical patient populations. As a member of a multidisciplinary health team, the Social Worker Case Manager acts as a liaison between the patient and community resources/post-acute care providers. They create and facilitate the timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. In addition, they facilitate patient movement across the continuum of care.

As a Social Worker Case Manager PRN, you will assist our medical patients in finding the services they need for continuity of care upon leaving the hospital. Our team collaborates closely with our community partners to provide services such as home health, hospice, skilled nursing, assisted living, private care, and durable medical equipment (DME).

What you will do in this role:

  • You will collaborate with the health care team and develop treatment goals and plan intervention that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, engagement in high risk social behaviors, limited finances and/or physical disabilities.

  • As a member of Case Management team you will provides crisis and supportive counseling to enhance the patient’s and family’s problem solving and coping skills, and counsel patient/families regarding advance medical directives.

  • You will document patients’ social work assessments, treatment goals, interventions, outcomes, and discharge plans.

  • You will refer patients who require extended care post discharge to long-term care facilities, and once an appropriate bed is located, arrange for the patient’s transfer.

  • You will identify, track, and record Alternative Care Days (ACDs), intervene to prevent or decrease ACDs as appropriate.

  • You will perform admission and concurrent review for all payers and maintain legible documentation as directed by the Utilization Management Plan.

What qualifications you will need:

  • Graduate of an accredited school of social work required.

  • Bachelor degree in Social Work BSW required (MSW preferred).

  • Licensed Clinical Social Worker preferred.

  • 2 or more years of medical social work experience in an acute care setting preferred.

  • Previous case management or utilization review experience preferred.
