Position SummaryThe Case Manager provides intensive, client-centered case management to individuals engaged through the OASAS Street Outreach program. This role focuses on reducing barriers by addressing health, behavioral health, social-service, and legal needs through comprehensive assessment, service planning, warm hand-offs, and ongoing follow-up. Key Responsibilities * Conduct initial biopsychosocial assessments to identify needs, goals, preferences, and barriers. * Develop individualized service plans and update them regularly. * Provide direct linkages to ACQC internal programs, including: * Mental health treatment (MHOTRS clinic) * Medication Assisted Treatment for Substance Use Disorders (SUD) * Housing services * Legal services * Health Homes/medical case management * Refer clients externally to medical, behavioral, and social-service organizations as appropriate. * Accompany clients to appointments when needed. * Provide advocacy to ensure clients remain engaged in services. * Maintain ongoing communication to ensure continuity of care. * Track referral outcomes and ensure service plan progress. * Maintain accurate, timely electronic records of assessments, plans, progress notes, referrals, and outcomes. * Work collaboratively with the Outreach Specialist and Peer Specialists to ensure coordinated client support. * Participate in scheduled team meetings and ACQC-required training. * Other duties as assigned by the Program Supervisor or Harm Reduction Manager Qualifications & Requirements * Bachelors degree in social work, Human Services, Psychology, or related field. * Minimum 4 years of professional experience serving people who use drugs (PWUD), people living with HIV (PLWH), people experiencing homelessness, or individuals with substance use disorders (SUD) or covered outpatient drug (COD) use. * Strong familiarity with NYC social-service systems, housing, benefits, and harm reduction principles. * Bilingual preferred, English/Spanish. * Must be culturally competent and able to work with individuals from non-traditional backgrounds in terms of gender identity and sexuality. Must be able to convey sensitivity in a non-judgmental manner. The Case Manager position is a full-time, non-exempt position, based in Richmond Hill, Queens, near the E train and LIRR. The successful candidate will play a vital role in coordinating care and support services for clients living with or at risk for HIV/AIDS and related conditions.
