How you’ll make an impact in this role

  • Design Integrated Care Plans: Create comprehensive care pathways that honor patient values and family preferences, ensuring every individual feels seen and heard throughout their recovery.

  • Champion Patient Advocacy: Actively identify and remove systemic barriers to care, serving as a dedicated advocate to ensure patients receive the resources they deserve.

  • Drive Clinical Continuity: Lead ongoing collaborations with supervisors and transition teams to adapt health plans in real-time, ensuring seamless shifts between care settings.

  • Consult on Documentation Excellence: Serve as a strategic resource for the clinical team, guiding accurate medical record documentation to ensure patients qualify for the highest standard of post-discharge services.

  • Evaluate Medical Necessity: Conduct precise clinical assessments to monitor level-of-care requirements, ensuring our medical interventions align perfectly with patient needs.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required . Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

  • BLS Provider credentialed from the American Heart Association (AHA) preferred.

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • Diploma from an accredited school/college of nursing required.
