Job Description
In this role, the RN Case Manager will support adult and pediatric inpatient units and the Adult Emergency Department by coordinating patient care activities based on established provider care plans. You will manage a daily caseload of hospitalized patients from a care-coordination perspective, working closely with physicians, bedside nursing staff, social work, and interdisciplinary teams to support appropriate admissions, continued stays, and safe, timely discharges. This position focuses on active inpatient care coordination, identifying barriers to progression early, aligning care plans with clinical and regulatory guidelines, and facilitating post-acute services such as rehab, skilled nursing, or home health as needed. This is a non-bedside, non-treatment role that does not involve direct patient care, hands-on clinical tasks, or responsibility for patient outcomes.
Responsibilities
Coordinate patient care activities for adult and pediatric inpatient units and the Adult Emergency Department.
Manage daily caseload of hospitalized patients from a care-coordination perspective.
Collaborate with physicians, nursing staff, social work, and interdisciplinary teams.
Support appropriate admissions, continued stays, and safe, timely discharges.
Identify barriers to progression early and align care plans with clinical and regulatory guidelines.
Facilitate post-acute services such as rehab, skilled nursing, or home health as needed.
Participate in interdisciplinary rounds and apply established care criteria.
Document all case management activity in Epic.
Support patient flow and transitions of care in a high-acuity, Level I Trauma Center environment.
Essential Skills
Oregon RN License.
Bachelor's Degree in Nursing (BSN).
Basic Life Support (BLS) certification through the American Heart Association.
1+ year of inpatient case management experience within the last 5 years.
Experience in an acute hospital setting.
Strong experience in Emergency Department and/or ICU preferred.
Demonstrated professionalism, excellent communication, and reflection skills.
Ability to work collaboratively on a team, provide and receive feedback, delegate, prioritize, and adapt in stressful situations.
Additional Skills & Qualifications
Epic experience preferred.
Travel experience preferred.
Work Environment
The work environment includes short-term acute care in the Care Management Unit, with a schedule of Monday to Friday, 7:00 AM-3:30 PM. The role involves floating to adult inpatient, Emergency Department, and Pediatrics, with variable weekend coverage and the expectation of up to one weekend shift per pay period. There is no on-call requirement. The patient population includes neonates, infants, adolescents, adults, and geriatrics. The typical caseload ratio is 1:20–25, with a day shift ratio of 1:25 and weekend ratio of 1:75. Parking costs require a $16 daily pass, and public transportation is highly encouraged. The scrub color can be any color except prints.

Pay and Benefits
The pay range for this position is $45.00 - $62.00/hr.
