Penn Medicine Princeton Health has an exciting opportunity for a clinical professional to join our team as the Program Lead CMS TEAM (Centers for Medicare and Medicaid Services Transforming Episode Accountability Model). If you are a clinician who has experience working with case management or population health management, this could be the new position for you!

This role serves as the physician-aligned operational extension of the CMO specifically for Penn Medicine Princeton Health's CMS TEAM participation and is not an enterprise-wide Penn Medicine role.

Role Summary- The Program Lead is responsible for local execution of the CMS Transforming Episode Accountability Model (TEAM) at Penn Medicine Princeton Health.

  • This role is accountable for acute and post-acute length of stay optimization and patient experience across the full 30-day episode of care for TEAM-eligible services at Princeton Medical Center.

  • The position focuses exclusively on Princeton Health operations and affiliated post-acute partners.

Scope- CMS TEAM participation for Penn Medicine Princeton Health only- TEAM-eligible surgical episodes at Princeton Medical Center- Cross-continuum operations from index admission or procedure through 30 days post-discharge

  • Post-acute partners, including skilled nursing facilities and home-based care supporting Princeton Health patients. This role emphasizes program-level coordination, communication, and enablement across multiple teams using existing workflows and clinical systems. The Program Lead leverages Epic functionality and related clinical information tools to improve visibility, information flow, and alignment across acute care, orthopedics, care management, and post-acute partners.

Key Responsibilities:

Physician-Led TEAM Governance- Serve as the CMO’s operational delegate for CMS TEAM performance at Princeton Health

Partner with Princeton-based surgical, hospitalist, and procedural physician leaders

Prepare TEAM performance updates for Princeton Health medical staff and quality committees.

Facilitate work-flow development and PDSA cycles for continuous process improvement

Develop meeting agendas, coordinate materials and logistics (including invitations and minutes), and actively participate in CMS TEAM Governance meetings Acute Care LOS Optimization

Provide program-level oversight of acute LOS performance for TEAM episodes by synthesizing existing case management, utilization review, and service-line data to identify systemic barriers and variation.

Escalate physician-dependent barriers through Princeton Health CMO-led governance SNF and Post-Acute LOS Leadership

Assist in the design of post-acute LOS Strategy, and be accountable to execution of such strategy

Coordinate program-level performance management of preferred SNF partners for TEAM patients, including LOS trends, outcomes, and escalation of recurring issues. Leads the Skilled Nursing Facility High

Performing Network, including discussing performance data, metrics, and patient complaints as needed with other ad hoc team members, such as Care Coordination- both IP and Ambulatory.

Hold in-person meetings with SNF preferred network facilities in an effort to support and sustain engagement.

Conduct joint case reviews on prolonged SNF LOS, readmissions, and placement delays

Establish new SNF partnerships where needed.

Patient Experience Across the Episode- Ensure patient and family expectation-setting aligned with Princeton Health workflows

Address LOS- and discharge-related patient experience issues Quality, Safety, and Outcomes

Monitor key CMS and Penn Medicine defined quality metrics - including readmissions, complications, and ED utilization to evaluate Princeton Health's TEAM episode performance.

Lead structured review of high-cost or poor-outcome cases

Analytics and Performance Management- Maintain a Princeton Health–specific TEAM dashboard and periodically QA review EPIC to ensure accuracy

Provide service-line and surgeon-level transparency for Princeton Medical Center- Ensure beneficiary-identifiable data from CMS is requested/received through annual submission of DSA and a DRA forms- Maintain active presence in TEAMS portal

Credentials:

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)

  • Education Specialization: Nursing, PT, OT, ST, Healthcare Management or Related Field. Will also accept Master’s in Social Work.

  • Equivalent Experience: •3+ years clinical experience

Salary Range:

$93,859.58 - $136,355.23/Annually based on 40 hours per week
