Squirrel Hill Health Center Substance Use Disorder Program Coordinator in Pittsburgh, Pennsylvania

Squirrel Hill Health Center is hiring a Substance Use Disorder (SUD) Program Coordinator to support recovery, improve health outcomes, and sustain public health initiatives like harm reduction and medication-assisted treatment (MAT). ​ This role serves as a liaison between patients, providers, staff, and community partners, while coordinating all aspects of the SUD program. ​

Responsibilities: ​

  • Coordinate the SUD/MAT program and track patient progress. ​

  • Conduct patient assessments, create care plans, and provide education on harm reduction and overdose prevention. ​

  • Facilitate referrals to behavioral health, counseling, and social services. ​

  • Maintains program standards of COE, MAT and SUD programming, including workflows and documentation for providers and patients.

  • Collects, tracks, and reports program data in REDCap and other reporting systems. Prepares reports for MCOs, PittPERU, funders, and accrediting agencies.​

  • Support Mobile Unit visits, including check in and appointment scheduling.

  • Build relationships with external providers and agencies to support patient care. ​

Qualifications: ​

  • Bachelor’s degree in social work, public health, counseling, nursing, or related field (MSW preferred). ​

  • Experience in substance use disorders or behavioral health preferred. ​

  • Knowledge of MAT, harm reduction, medical terminology, and EHR systems. ​

  • Strong organizational, communication, and documentation skills. ​

  • Proficiency in Microsoft Office/Google Suite; REDCap experience preferred.

  • Bilingual ability is a plus. ​

  • Commitment to serving underserved populations and aligning with the SHHC mission.
