Job Title: Medical Assistant - Population HealthJob Description

The Population Health Medical Assistant supports primary care teams within the network, either remotely or embedded within a primary care office, to enhance compliance with population health initiatives such as annual wellness visits, quality care gap closures, and in-network referrals. This role does not involve direct patient care but requires executing standing orders for patients.

Responsibilities

  • Review patient charts regularly to identify screening needs and quality gap closures, as well as care coordination requirements.

  • Work under appropriate physician licensure to use standing orders developed by primary care physicians for screening and quality gap closure needs.

  • Prepare charts before office visits for value-based patients to identify open care gaps.

  • Manage in-network referrals for value-based patients.

  • Perform member outreach to schedule patient visits for campaigns such as annual wellness visits, well child visits, breast cancer screenings, and colorectal cancer screenings.

  • Coordinate follow-up visits for transitions of care for value-based patients.

  • Transfer patient data regarding quality care gaps from clinic medical records to population health management platforms.

  • Proactively work with payer care gap and high-risk reports for value-based patients.

  • Collaborate with primary care teams and clinical performance consultants to effectively manage value-based patients.

  • Fully disclose relevant training, experience, and credentials to help patients understand the scope of services provided.

  • Work collaboratively with teams in physician offices and assigned support teams, following instructions, analyzing data, and coordinating tasks to meet business goals.

Essential Skills

  • Proficiency with electronic health records and appointment scheduling systems.

  • Experience with executing standing orders and patient outreach for wellness visits and screenings.

  • Bilingual in English and Spanish.

  • Three years of experience as a Medical Office Referral Coordinator, Medical Assistant, or in a similar role within a Health Plan or Managed Care Organization.

  • Diploma from a Medical Assistant program.

  • Familiarity with CPT codes, especially CPT-2 codes.

  • Experience with Microsoft Office Suite, including Outlook, Excel, and calendar management.

  • Knowledge of quality measures such as care gap closures, pre-visit planning, HEDIS measures, and patient outreach.

  • Excellent verbal communication skills.

Additional Skills & Qualifications

  • Ability to follow instructions, analyze data, and manage workload appropriately to meet business goals.

Work Environment

The role is based out of a central hub but involves floating to different provider offices across Maricopa County. The work schedule includes working from the office in Arcadia on Mondays, visiting clinics in the assigned territory from Tuesday to Thursday, and possibly working at a clinic or from home on Fridays, depending on clinic hours.

Job Type & Location

This is a Contract to Hire position based out of Phoenix, AZ.

Pay and Benefits

The pay range for this position is $22.00 - $28.00/hr.
