SUMMARY STATEMENT: The Direct Care Workeris responsible for providing personal care services as prescribed according to written directions/care plan.
DUTIES AND RESPONSIBILITIES:
- Follow client care plan.
- Maintain client confidentiality and privacy and treat all clients with dignity and respect.
- Perform duties in accordance with Agency policies and procedures and applicable Federal, State, and Local regulations.
- Attend inservice programs as required by Federal, State, and Local regulations and agency requirements.
- Provide personal care, rehabilitative, comfort and supportive services essential to the clients health.
- Assist in activities of daily living (personal hygiene, grooming, eating, dressing, elimination, exercise and ambulation.
- Prepare and serve basic diet.
- Assist with range of motion, mobility, transfers, toileting and specimen collection, as applicable.
- Maintain a safe environment.
- Assist with self administered medications.
- Support the client and maintain the appropriate setting to encourage independence.
- Participate in coordination of client services.
- Participates in office in-services and meetings as requested by the Home Assist Manager.
- Prioritize and schedule duties to ensure all client care tasks and visits are completed as assigned.
- Communicate with the Agency weekly and as needed regarding scheduling, availability, attendance and assignment, according to established policy and procedure.
- Utilize standard precautions and infection control techniques as necessary.
- Consistently demonstrate competency of all skills included in DCWscope of practice recognized by Agency.
- Perform other duties as assigned.

Qualifications
Education and Experience:
- Direct Care Worker certification
- 1 year experience as a PCA/DCW.
- CPR certification.
- First Aide certification.
- Fingerprint Card
Specific:
- Access to utilization of public or private transportation system.
- Ability to understand and carry out directions or instructions and to keep simple records in English.
- Ability to demonstrate competency in all skills included in DCWscope of practice recognized by Agency.
- Ability to use the telephone; good communication skills.
- Ability to communicate information concerning a clients condition clearly.
- Ability to lift 30 pounds, move intermittently throughout the work day, inclusive of sitting, standing and bending, pushing, pulling, or moving objects and clients.
- Ability to relate to and work with the various personality styles, ill, disabled, and elderly.
- Skilled in interpersonal relationships.
- Has a caring attitude.
