SUMMARY OF JOB
In conjunction with Director of Nursing, provides direct supervision to the home care team. Delivers quality patient care including coordinating, implementing and evaluating home care services as evidenced by positive outcomes. Assumes 24-hour accountability for patient care functions and the direction of the patient care team. Contributes to the attainment of ministry goals by assisting in developing the operational plan and budget. Assures agency compliance with local, state, federal, government regulations and ACHC standards. Functions as a liaison in the community articulating and reinforcing the Redeemer Health Home Care and Hospice story and value of our service. Develops and maintains collaborative relationships with internal and external customers including employees, physicians, facilities and community members.

RECRUITMENT REQUIREMENTS
BSN and/or degree in health-related field preferred. Graduate from an accredited school of nursing.
Two years clinical experience. Minimum three to five years home care experience preferred.
Current supervisory experience preferred.
Ability to function in a rapidly changing health care environment.
Excellent communication skills, customer service skills, and positive leadership style.
Proven Computer literacy
Ability to travel to all Redeemer Health entities, and to patients’ homes within service area.

LICENSE AND REGULATORY REQUIREMENTS:
Current licensure in State of practice, Valid Driver’s License, Current CPR Certification
