Job Description
Social Service/Admissions Director
Job Function: Completion of Admission and Discharge Planning Process, Delivery of all other Social Service functions\ \
Supervises: Assistant as applicable
Reports To: Administrator
Primary Responsibilities:
- Assist the Marketing Director in management of referrals.
- Complete facility tours.
- Complete admission paperwork and paperwork related to change in payor\ source
- Serve as a liaison between facility, residents, responsible parties, and
outside agencies
- Facilitate the discharge planning process; development and implementation of
discharge care plans.
- Refer resident to outside agencies as appropriate.
- Provide information to resident/responsible parties as to Medicare/Medicaid,\ and other financial assistance programs available to the resident in conjunction\ with the Business Office Manager.
- Keep abreast of current state and federal regulations, professional standards,\ and facility policy and procedures.
- Attend meetings as required.
- Facilitate grievance process in conjunction with the Grievance Officer
- Provide social support for residents as needed.
- Complete observations and additional documentation as required.
- Complete MDS sections as assigned, ensure all areas are addressed in the\ care plan as necessary.
- Provide assistance to Activity Director in addressing mood and behavioral issues.
- Participate as a member of the Interdisciplinary Team.
- Perform other duties as assigned by the supervisor.

- Complete admission paperwork and process
