Position Summary: The Family Engagement Coordinator is a critical member of the residential and community residential program. The Family Engagement Coordinator's focus is on family voice and choice. The Family Engagement Coordinator is responsible for developing the after-care plan starting with the youth's placement on the dorm and focusing on permanency.
Responsibilities:
Meet families in their homes and community
Schedule home visits with families, write visitation plans, schedule transportation with RTC Campus Supervisor, and complete before and after notes in connections
Focus on Permanency activities
Ensure family voice and choice is evident in ICSP development, treatment plans goals, meetings,
Trained in Child Adolescent Needs and Strengths Assessment (CANS) and gather information for Social Worker
Gather information from family for 30 psychosocial
Attend psychiatrist meeting if Social Worker is not available
Provide casework services related to discharge planning and the youth's re-entry into the family and community
Develops transition plans with and for the youth, including education/vocational/life skills. Coordinate with the Aftercare program.
Support the Social Worker in identifying permanency barriers, kinship resources, and visiting/mentoring opportunities early in placement
Train family/discharge resource in skills needed to sustain gains made in care
Plan, invite, and transport family for events and meetings
Attending staff meetings to help with dorm communication
Complete and enter all notes in EVOLVE, including but not limited to: TTH's, collateral contacts, case contacts, phone calls & emails, all within 14 days.
Requirements
Education (one of the following):
Associate's degree required
Bachelor's degree preferred
Relevant experience may be considered in lieu of the degree requirement
Experience:
2 years of related experience required
Driver's License and ability to maintain insurability throughout employment required
