As a Hospice Home Health Aide, you will provide essential, hands-on care, ensuring dignity and well-being for our patients. Under the guidance of a Registered Nurse, your daily impact will include:
Personal Care & Comfort: Assisting with personal hygiene, mobility (ambulation and transfers), and promoting overall patient well-being.
Patient Monitoring: Diligently reporting any changes in condition to the nursing team.
Supportive Environment: Performing light housekeeping, preparing meals, and ensuring a safe, healthful, and comforting home environment.

Job Requirements
High School Diploma or equivalent required.
Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required.
Basic Life Support (BLS) required.
Must have a valid Driver’s License.
Must have current vehicle insurance.
